Create and Submit a form

You can either fill the form online by clicking on Create on the left navigation bar, or fill it off line downloading a .doc version of the html form.

If you fill the form online:
You can either go all the way through and submit the form.
Or Stop at some point, save what you did on our server and finish later using your personal login and password in the Modify section . Once you are done you can submit your form

If you fill the form offline:
Download the form, fill it with Microsoft Word. Save your modifications on your own disc. Once you are done, save a copy as html, open it with your browser and submit your form online.

In any case you will first preview the result of your form online, then you will be offered the choice of submitting or saving it.

Once Submitted it takes a few days for the form to appear online since we need to check that it does not contain illegal information or virus.

Modify an existing Form

If you wish to modify a form that you have saved and not submitted yet, go to the Modify Section, enter your personal info and work on your document
If you wish to add comments or contacts to an already submitted form (wether you created it or not), just open the form and press on Add comments.
